PDF to Image renders whole pages as pictures — text and all — at a chosen DPI. Extract Images pulls out the original embedded photos at their true resolution, which is usually far higher than a page render. For getting photos out of a report, extraction wins every time.
Whatever they were stored as inside the PDF — typically JPEG or PNG. No re-compression happens on the way out, so quality is exactly what the document author embedded.
Your PDF is probably a scan: each page is one full-page image rather than separate embedded photos. For scans, use PDF to Image to render the pages, or OCR to get the text.
